BMC and the Y Partner to END Diabetes with grant from AstraZeneca
Boston Medical Center, in partnership with the Y, received just 1 of only 14 grants nationally from the AstraZeneca Health Foundation’s Connections to Cardiovascular Care Initiative in the amount of $218,412 to provide the END Diabetes Program at our Dorchester, Roxbury, East Boston and Huntington Ave Y's.
END Diabetes is a 6 month community program intended to improve cardiovascular health by reducing the risks of diabetes and obesity in individuals who have been diagnosed with Type 2 diabetes and to prevent diabetes in those who are at risk of developing the disease. The 6-month program begins with educational workshops, followed by an intensive 12-week program curriculum that includes individual coaching and regular exercise at the Y during which participants receive a free 6 month membership. With AstraZeneca funding, we will serve 200 adults in the first year of END Diabetes.
The END Diabetes program will show that with lifestyle change and modest weight reduction, our population will reduce their risk of cardiovascular disease.
